硬腦膜外血腫（Epidural hematoma），簡稱硬膜外血腫，是指覆蓋大腦的堅韌外膜（硬腦膜）之外與頭骨之間的出血^[4]。頭部受傷後，患者常常會失去意識（英語：Unconsciousness），短暫恢復意識，之後再次失去意識^[2]。其他症狀包括可能頭痛、精神錯亂、嘔吐以及無法移動身體的某個部位^[1]。併發症包括可能癲癇發作^[1]。

典型的病因是頭部損傷，造成顳骨骨裂和中腦膜動脈（英語：Middle meningeal artery）出血^[4]。凝血病或血管崎形（英語：Vascular malformation）也可能是病因^[1]。通常透過計算機體層成像或磁共振成像等影像檢查確定診斷^[1]。若類似的出血發生在脊柱時，就稱為脊椎硬膜外血腫（英語：Spinal epidural hematoma）^[4]。

硬腦膜外血腫一般以開顱手術或鑽孔手術等緊急手術治療^[1]。文獻中也曾提過利用植入骨內針來引流出血，達到減壓的目地^[5]。若不治療，通常會死亡^[6]。在頭部損傷患者中約1% 到 4%會發生硬腦膜外血腫^[1]。通常發生在年輕人^[1]。男性較女性多^[1]。
