光化性角化病（英語：actinic keratosis，縮寫：AK）又稱為日光性角化病或日光性角化症，是一種皮膚癌前狀態（英語：Precancerous condition）的病變，之後可能發展為皮膚鱗狀細胞癌，但也可能不治療就自癒[1]。通常成群出現在頭部、手臂和手背等經常日哂的部位。邊界清晰，呈紅色、皮膚色或色素沈澱（英語：Hyperpigmentation），質地堅硬或呈現疣狀、光滑、有光澤或鱗狀丘疹[2]。單一病兆的大小常在0.2至1公分間[1]。肥厚型可能形成皮角[2]。通常病兆沒有任何症狀，但若被抓傷，可能會出血[3]。

病因是長期暴露在紫外線下[4]。危險因子包括淺膚色、日曬較多、免疫功能差、接觸砷[2][1]。可能的致病機轉為紫外線對脫氧核糖核酸造成損傷。通常由皮膚的外觀診斷；但診斷不明的病例，也可進行活體組織檢查或切除後再進行組織學檢查[5][2]。鑑別診斷包括狼瘡、落葉型天疱瘡、頭皮糜爛性膿疱性皮膚病（英語：Erosive pustular dermatosis of the scalp）和基底細胞癌[2]。

光化性角化病有多種療法[5]。通常以冷療治療，治癒率約為75%；然而，可能造成皮膚不均勻地異外變白[6][7]。若病兆面積大，外用藥膏（如5-氟尿嘧啶或咪喹莫特（英語：Imiquimod））的效果可能更佳[8][9]。光動力療法是治療多處病兆的另一種選擇[10]。若不及時治療，8-20%的患者將進展為皮膚鱗狀細胞癌[6][11]。

光化性角化病很常見[10]。男性比女性更容易發病。疾病的盛行率會隨著年齡而增加，好發在50歲以上[1][2]。若其他因素相同時，赤道附近的發生率較高[12]。光化性角化病是由威廉·杜佈雷伊（英語：William Dubreuilh）（William Dubreuilh）在1896年描述[13]。現在用的名稱由平庫斯（Pinkus）於1958年提出[12]。

體徵和症狀

光化性角化病最常見的症狀是出現厚度不等的白色鱗狀斑塊，周圍為紅色；用戴手套的手觸摸時，會感受到砂紙般的質地。病變附近的皮膚常常也有日光造成損傷的痕跡，典型表現為色素改變，在色素沉著的區域同時又有黃色或蒼白變化；皺紋變深、紋理粗糙、紫癜和瘀斑、皮膚乾燥（英語：Xeroderma）和散佈其間的毛細血管擴張[14]。

光照老化會積累致癌的變化，引起突變的角質細胞增生，並可能表現為光化性角化病或其他的表皮腫瘤[15]。由於長年累積的日照傷害，皮膚單一區域內可能會出現多處光化性角化病。這種情況稱為區域癌變。

病兆通常沒有症狀，但可能會有疼痛、瘙癢、出血或產生刺痛或灼熱感[16]。通常根據臨床表現進行分級：I級（容易看到，稍微可觸），II級（容易看到並觸摸到），III級（明顯看到且過度角化）[17]。

若皮膚病兆出現潰瘍、結節或出血（英語：Friability）時應特別小心變成惡性腫瘤[18]。具體來說：易惡化成皮膚鱗狀細胞癌的高風險臨床表現，可簡化為英文縮寫「IDRBEU」[19]：

  • I（induration/inflammation）：硬化／發炎
  • D（diameter）：直徑>1公分
  • R（rapid enlargement）：快速變大
  • B（bleeding）：出血
  • E（erythema）：紅斑
  • U（ulceration）：潰瘍

光化性角化病通常依臨床變化診斷，但臨床上單依外觀，有時很難與鱗狀細胞癌區分，若出現以上特徵，建議進行活檢以確定診斷[20]。
