光化性角化病（英語：actinic keratosis，縮寫：AK）又称为日光性角化病或日光性角化症，是一种皮肤癌前狀態（英语：Precancerous condition）的病变，之後可能发展为皮膚鱗狀細胞癌，但也可能不治疗就自癒[1]。通常成群出现在头部、手臂和手背等經常日哂的部位。边界清晰，呈红色、皮肤色或色素沈澱（英语：Hyperpigmentation），质地坚硬或呈現疣狀、光滑、有光泽或鳞状丘疹[2]。单一病兆的大小常在0.2至1公分间[1]。肥厚型可能形成皮角[2]。通常病兆没有任何症状，但若被抓伤，可能会出血[3]。

病因是长期暴露在紫外线下[4]。危险因子包括淺膚色、日晒较多、免疫功能差、接触砷[2][1]。可能的致病机轉為紫外线对脱氧核糖核酸造成损伤。通常由皮膚的外观诊断；但诊断不明的病例，也可进行活體組織檢查或切除後再进行组织学检查[5][2]。鑑別診斷包括狼疮、落葉型天疱瘡、頭皮糜爛性膿疱性皮膚病（英语：Erosive pustular dermatosis of the scalp）和基底細胞癌[2]。

光化性角化病有多种疗法[5]。通常以冷療治療，治愈率约为75%；然而，可能造成皮肤不均勻地異外变白[6][7]。若病兆面积大，外用药膏（如5-氟尿嘧啶或咪喹莫特（英语：Imiquimod））的效果可能更佳[8][9]。光動力療法是治疗多处病兆的另一种选择[10]。若不及时治疗，8-20%的患者將進展为皮膚鱗狀細胞癌[6][11]。

光化性角化病很常见[10]。男性比女性更容易發病。疾病的盛行率會随着年龄而增加，好发在50岁以上[1][2]。若其他因素相同時，赤道附近的發生率較高[12]。光化性角化病是由威廉·杜佈雷伊（英语：William Dubreuilh）（William Dubreuilh）在1896年描述[13]。现在用的名称由平庫斯（Pinkus）于1958年提出[12]。

光化性角化病最常见的症狀是出現厚度不等的白色鳞状斑块，周围為红色；用戴手套的手触摸时，會感受到砂纸般的质地。病变附近的皮肤常常也有日光造成损伤的痕跡，典型表現為色素改变，在色素沉着的区域同時又有黄色或苍白變化；皱纹變深、纹理粗糙、紫癜和瘀斑、皮膚乾燥（英语：Xeroderma）和散佈其間的毛细血管扩张[14]。

光照老化会积累致癌的变化，引起突变的角质细胞增生，并可能表现为光化性角化病或其他的表皮肿瘤[15]。由于长年累積的日照伤害，皮肤单一区域內可能会出现多处光化性角化病。这种情况称为区域癌变。

病兆通常沒有症状，但可能会有疼痛、瘙痒、出血或產生刺痛或灼热感[16]。通常根据临床表现进行分级：I级（容易看到，稍微可触），II级（容易看到並触摸到），III级（明显看到且过度角化）[17]。

若皮膚病兆出現潰瘍、结节或出血（英语：Friability）時應特別小心變成恶性肿瘤[18]。具体來說：易惡化成皮膚鱗狀細胞癌的高风险临床表现，可簡化為英文縮寫「IDRBEU」[19]：

  • I（induration/inflammation）：硬化／發炎
  • D（diameter）：直径>1公分
  • R（rapid enlargement）：快速變大
  • B（bleeding）：出血
  • E（erythema）：红斑
  • U（ulceration）：溃疡

光化性角化病通常依临床變化诊断，但临床上單依外觀，有時很难与鳞状细胞癌区分，若出現以上特征，建議進行活检以確定診斷[20]。
